Through the partnership, Hackensack Meridian Health patients use connected devices at home to share vital signs such as blood pressure, weight, heart rate, and blood glucose. Cadence’s AI-powered Proactive Care Engine analyzes this data in real time, alerting the clinical team when intervention is needed and giving patients support with medications and lifestyle guidance. “Patients deserve care that continues between appointments – care that anticipates needs and prevents problems before they escalate,” said Dr. Anuj Mehta, chief clinical officer, Hackensack Meridian Health. “With Cadence, we are building the infrastructure for that kind of care.
